Perkins Guest Experience Survey:

On the welcome page of the website www.PerkinsExperienceSurvey.com, you will be required to enter for 15 digit survey code from bottom of your receipt and complete the online survey questions. Participants can enter in to the sweepstakes survey during the promotion period that commences at 12:00am Eastern Standard Time (EST) on January 25, 2010 and ends at 11:59pm EST on December 26, 2010.

After completing the survey, you are eligible to take part into the sweepstakes and win a grand prize. If your online survey is unfinished, then the entry will be invalid.

For the sweepstakes, other two ways are also available. People can also enter into the sweepstakes through telephone by calling 1-866-355-0311 and provide the code from your guest receipt and complete the short phone survey. Otherwise you can enter by hand-printing your name, address, day and night telephone numbers and age on a 3”x 5”card and mailing it in a standard business-size #10 envelope to: Perkins “Guest Experience Survey” 90 Rockwood Place, Rochester, N.Y. 14610. All mail-in entries must be postmarked by December 26, 2010 and received by January 2, 2011.

Who are eligible for the Perkins Guest Experience Program?

Only legal residents of the United States and Canada excluding Quebec are eligible for the sweepstakes survey, but entrance must be 18 years old or older at the time of entry. The officers, directors, employees and representatives of Sponsor, its parent company, affiliates, subsidiaries, licensees, service providers, advertising and promotion agencies and those who involved in the production, development, implementation or handling of the sweepstakes are not eligible to participate in the Sweepstakes as well as the immediate family members (parents, children, siblings, spouses) and/or persons living in the same household are also not eligible.

Prizes:

Total of eleven grand prize winners will be selected, one for every month of the Sweepstakes Period and each one will win a $500 cash award.
